Moonee Valley Melbourne Primary Care Partnership Service Coordination Care Planning Project 2004-2005The Care Planning Project will develop and trial a care planning protocol for HACC clients with complex needs who are using multiple services without receiving case management. The project will develop and trial care planning practices, processes, protocols and systems around how care plans are developed with consumers, carers and service providers and how consumers and carers are involved in decision making around their care plan. The development of an agreed understanding of the role of a "key worker" and the trialling of this is a central part of the project.
