Moonee Valley Melbourne Primary Care Partnership Service Coordination Care Planning Project 2003-2004

The Care Planning Project will develop and trial a care planning protocol for HACC clients with complex needs who are using multiple services without receiving case management. The project will develop and trial care planning practices, processes, protocols and systems around how care plans are developed with consumers, carers and service providers and how consumers and carers are involved in decision making around their care plan. The development of an agreed understanding of the role of a "key worker" and the trialling of this is a central part of the project.

Aims
Trial and implement a care planning protocol to improve service coordination and client outcomes

Project objectives: 1. Determine the role of the key worker; 2. Develop agreement about the nomination of a key worker and what core skills are required; 3. Develop practices, processes, protocols and systems around how care plans are developed with consumers/carers and service providers, and how consumers/carers are to be involved in decision making around their service coordination plan; 4. Ensure privacy requirements are met in regards to multidisciplinary care planning; 5. Develop practices, processes, protocols and systems around review schedules for service coordination plans, initiation of reassessments and providing feedback to referral agencies.
